Here is the excel sheet which includes the departments surveyed. The findings have been been removed, but the other other information is left as a sample. You will see that some departments have two worksheets, one with a (2) beside the name. These are clinical departments who are surveyed twice a year. The others with are non-clinical departments and only surveyed once a year. Once I have the tracking form filled in, I send out a work ticket or email to the appropriate department stating what needs to be done. I keep copies of these tickets/emails for my file. I also place a note on the bottom of the tracking form as to what I did. I then place the due date on my Outlook calendar and as a pop-up reminder. If I have not heard from them, another email is sent requesting an update. Once an "AFI" is completed, I mark it on the tracking form.
